Support for Transition-Age Patients
As young people with life-limiting or life-threatening conditions approach adulthood, the Sheffield Children’s Palliative Care Team helps ensure that care continues smoothly and support remains consistent.
Their role during this stage is to prepare and guide patients and families through the transition from paediatric to adult palliative or specialist care services.
What the Team Provides
- Early transition planning: The team starts conversations about transition well before the 18th birthday - often from around age 14 - to identify future care needs and preferences.
- Coordinated care and communication: They work closely with adult palliative care services, GPs, and other specialists to ensure there’s a clear and supported handover. This includes sharing the advance care plan and key medical information so that adult teams understand the young person’s condition and wishes.
- Symptom management and emotional support: The team continues to manage symptoms, review treatment plans, and support emotional wellbeing during the transition period.
- Empowering young people: Where appropriate encourage young people to take a more active role in their care - helping them understand their condition, treatment options, and how to make decisions about their future care.
- Practical guidance: Support with navigating adult healthcare systems, social care, education, and community services, helping families understand what to expect after transfer.
- Partnership working: Collaboration with organisations such as Bluebell Wood Children’s Hospice, and local adult hospices ensures ongoing, age-appropriate support beyond paediatric services.
