What the Palliative Care Team Does
The Sheffield Children’s Palliative Care Team supports children and families living with long-term, life-limiting, or life-threatening conditions.
They provide care both in hospital and in the community, focusing on improving quality of life and supporting families through all stages of illness.
Their work includes:
- Symptom management – helping control pain and other symptoms.
- Preventing unnecessary hospital admissions – by assessing and treating children at home when possible.
- Advance care planning – supporting families to make informed decisions about care preferences and future planning.
- Partnership working – collaborating with Bluebell Wood Children’s Hospice and other services.
- Coordinating multidisciplinary care – bringing together different specialists involved in a child’s care.
- End-of-life care and bereavement support.
- Home visits – by a palliative care consultant and nursing team.
How to Make a Referral
Eligibility:
- Referrals can be made for children and young people up to 19 years old who have a life-limiting or life-threatening condition.
- We encourage early referrals - ideally as soon as a life-limiting condition is identified and the family feels ready - so that the team can build relationships, provide support, and plan care proactively rather than waiting until a crisis arises.
How to refer:
Referrals are usually made by your lead consultant or another healthcare professional involved in your child’s care. They can contact the Sheffield Children’s Palliative Care Team directly to discuss the referral and care needs: scn-tr.palliativecareteam@nhs.net.
Community care:
- The team can provide clinical care at home for children and young people who are registered with a Sheffield GP practice.
- Families living outside Sheffield can still access the regional palliative care clinic for specialist support and advice.

Education and Training
High-quality palliative care depends on confident, well-supported professionals. Across South Yorkshire, we offer a range of education and training opportunities to help staff develop knowledge, skills, and confidence in caring for children and young people with life-limiting and life-threatening conditions.
Our training offer is open to staff working in hospitals, community services, primary care, and hospices, and is designed to support shared learning and collaboration across the region.
What We Offer: -
- Core and specialist palliative care training for doctors, nurses, and allied health professionals – including symptom management, communication skills, advance care planning, and end-of-life care.
- Study days and workshops hosted by Sheffield Children’s Hospital, Bluebell Wood Children’s Hospice, and community teams, focusing on practical approaches to complex care.
- Bespoke education sessions delivered to local teams on request, tailored to service or learning needs.

Education is open to professionals across Sheffield, Rotherham, Doncaster, and Barnsley, with both face-to-face and online learning options available.
