Doncaster Specific Palliative Care
Doncaster Paediatric Palliative Care Service
Nursing and Consultant support for children with complex palliative care needs, including home visits, symptom management, Advance Care Planning (ACPs), and MDTs.
Specialist support from Consultant and nurse from Sheffield Children’s Hospital visit to Doncaster monthly.
Referral: Clinician referral via phone/email to CCN Team and/or Dr Emad Armanious

Scope
Clinical setting: Community; appointments may take place in the child’s home, school, children’s centre or in a children’s hospice or hospital if the child has been admitted.
Catchment area: Infant/child/young person is resident in area covered by DBTH hospital service.
Age range: 0 – 19th birthday. Referrals are also encouraged to begin planning care and support for family during pregnancy when there is an antenatal diagnosis of a life-limiting condition
Hours of Service: Monday – Friday 08.00 – 18.00
Clinical Processes
Inclusion criteria:
We are able to offer support for the families of infants, children and young people who meet the following criteria:
- The infant, child or young person has a life limiting or life-threatening condition in accordance with the Together for Short Lives definitions of 'life-limiting/shortening' and 'life-threatening' conditions.
- The infant, child or young person is mainly cared for at home or discharge is being planned.
- We accept referrals for CYP in the catchment area from local paediatric teams, GP, community nursing team and tertiary care clinicians.
