South Yorkshire Children’s Palliative Care System Overview
The South Yorkshire (SY) paediatric palliative care system provides coordinated, compassionate support for children and young people with life-limiting or life-threatening conditions, and their families.
The aim is to deliver consistent, high-quality care across hospital, community, and hospice settings - supporting symptom control, emotional wellbeing, and quality of life.
South Yorkshire Children and Young People's Palliative Care Model
In response to national priorities and local needs, the South Yorkshire Children and Young People’s (CYP) Alliance has worked with partners across health, hospice, and community services to develop a joined-up system for palliative and end-of-life care. Designed in collaboration with clinicians and families, the model follows the principles of the Ambitions Framework for Palliative and End-of-Life Care, focusing on shared decision-making, consistent support, and seamless care across settings.
This model brings together expertise from across the region, including:
- A Specialist Paediatric Palliative Care Consultant providing system-wide oversight and expert advice.
- A dedicated specialist nursing team from Sheffield Children’s NHS Foundation Trust and enhanced community nursing roles in Barnsley, Rotherham and Doncaster to strengthen care at home.
- Local Lead Consultants in Rotherham, Doncaster, and Barnsley offering clinical leadership closer to home.
- Bluebell Wood Children’s Hospice, which provides respite, symptom management, end-of-life care, bereavement support, and family-focused services.
Together, these teams ensure that children, young people, and their families in South Yorkshire receive compassionate, coordinated care and support when they need it most.
