Each child has a named Community Children’s Nurse (CCN) who supports the child and family at home, school, or hospital.
Service runs 7 days a week (8am–8pm).

Barnsley Paediatric Palliative Care Service
Nursing and Consultant support for children with complex palliative care needs, including home visits, symptom management, Advance Care Planning (ACPs), and MDTs.
Specialist support from Consultant and nurse from Sheffield Children’s Hospital visit to Barnsley monthly.
Referral: Clinician referral via phone/email to Dr Claire Enevoldson.

Barnsley Adult Hospice
Supports anyone over 18 with a Barnsley GP. Works closely with families to transition care.
Referral: Clinicians, GPs or nurses via website form.

Family Hubs

What the service is

The Family Hubs are local community centres offering “joined-up early help services” for children, young people and their families in the Barnsley area. Specifically:

They cover children and young people from pre-birth up to age 19, and up to 25 years if the young person has a disability. They bring together a range of services - universal, targeted and specialist - including schools, social care, health services, the voluntary/private sector and adult services. The aim is to provide support at the right time and place, to help families with multiple and varying needs.

What the service is

Compass Be (MHST) works with children, young people and their families in the Barnsley area, specifically in education settings (primary/secondary schools) and community-based settings.  The support they provide is free and confidential for pupils/students, schools and families, covering emotional wellbeing and mild-to-moderate mental health needs.

Open Door to CAU (Children’s Assessment Unit)

Overview:
The Open Door system provides a pathway for children with complex or ongoing medical needs to be directly reviewed by the Children’s Assessment Unit (CAU) when concerns arise.

Referral process:

A clinician (such as a consultant, specialist nurse, or paediatrician) identifies that a child has complex needs and requests that they be added to the Open Door register.

Once a child is on this register, their parents/carers, Community Children’s Nurse (CCN), or GP can make a referral directly to the CAU for assessment.

To arrange a review, they simply phone the CAU ward directly - no new referral form or GP appointment is required.

Purpose:
This system ensures that children with complex or unstable medical conditions can be assessed quickly by the paediatric team, avoiding unnecessary delays or emergency department visits.
