How can I get a referral?
Schools support children and young people with additional needs through an ‘assess, plan, do and review’ graduated response process which means that early years services and schools will lead on the request for an assessment - not GPs.
Early years services and schools often know your child or young person best as they have valuable insights that can help clinicians make well-informed assessments. To support this, practitioners from the Healthy Families Team may also work closely with these settings to ensure your child receives the right care at the right time.
If you're concerned about your child or young person, the first step is to speak with the school's early years staff. They’ll look at your child or young person’s needs and work to put the right help and support in place. This includes gathering important health information at school to build a clearer picture. If concerns continue and a neurodevelopmental assessment is thought to be needed, the school team will refer your child to the neurodevelopmental pathway for further support.
What if my child isn't in an early years setting?
If your child or young person is not in an early years setting/school or is being educated at home, the Healthy Families practitioner will lead on submitting a request for an assessment.

Information, advice, and access to a range of support services is available for parents and carers of children with SEND needs without a diagnosis. Integrated Children’s Disability Service (ICDS)
The ICDS brings together colleagues from education and social care into one team to deliver a holistic approach to support for children and young people with disabilities aged 0 to 25 years.
There is a variety of support, advice and information available to help disabled children and their families with everyday tasks.
What is the process for referral?
At school the 'assess, plan, do and review' process will take place which includes the following steps:
- Identified need: What is the identified need of your child or young person and why would an assessment help?
- Service leads on assess, plan, do and review process: Engaging with family/carers and other professionals involved
- Service to complete two cycles of support plans over at least one school term
- Service to contact Healthy Families Team for the required health information

General Development Assessment
After the school completes the referral process, your child or young person will be referred to the community paediatrician who will complete a general development assessment.
Please note - inappropriate or incomplete referrals or those lacking evidence of pre-referral support and intervention, may be declined.
After completing the general developmental assessment, the paediatrician will decide which steps should be taken next to support your child or young person.
ADHD (Attention Deficit Hyperactivity Disorder) Pathway

ADHD (Attention Deficit Hyperactivity Disorder) Pathway
- If the community paediatrician doing general developmental assessment suspects ADHD, they will make a referral to Bassetlaw Multidisciplinary ADHD assessment team.
- The referral will be triaged by the ADHD assessment team and outcome of the triage will be communicated to parents, GP, School and the referring paediatrician.
- If the child is accepted for ADHD assessment, he / she will go on waiting list and family will be offered appointment in due course of time.
- If your child or young person is diagnosed with ADHD, he/she will be offered follow up appointment with his referring community paediatrician for consideration of medication. They will have a regular review, generally every 6 months.
- If your child or young person needs to continue ADHD medication into their adulthood, they will be transitioned to Adult ADHD service at the age of 18.
- In addition to medication, children and young people with ADHD will need to access help and support through school and community-based services.
Autism Pathway
- Should the community paediatrician undertaking the general development assessment suspect Autism, he / she will refer to multidisciplinary autism assessment team.
- The referral will be triaged by the autism assessment team and outcome of the triage will be communicated to parents, GP, School and the referring paediatrician.
- If your child or young person is accepted for Autism assessment, he / she will go on waiting list and family will be offered appointment in due course of time.
- If your child or young person receives an autism diagnosis, family and school will receive feedback and detailed report containing information on help and support. The child or young person will be discharged.
